INSTRUCTION GUIDE FOR RUSH 2nd U.S. PASSPORT
THE FOLLOWING DOCUMENTS ARE REQUIRED:
Proof of U.S Citizenship:
o Must submit your current and valid passport
o Must have an acceptable reason for requesting a second passport
Please note the following:
o If you lost your valid or recently expired passport, you must follow our instruction guide for "Replacing Lost or Stolen Passports"
o If you have a recently expired passport, you must follow our instruction guide for "US Passport renewal"
Photographs:
o Two recent original passport size quality photos
o Must be 2"" x 2"" color only
o No black & white photos
Proof of Departure:
o Copy of airline tickets or travel agent itinerary
o Very Important! Without proof of travel you will not be considered an expedite case
o The State Department expedites passports based on travel date or need for travel visas.
Government fees:
o Adults $170.00 payable to DEPARTMENT OF STATE
Letter of Authorization:
o You must complete and include one original letter of authorization signed in Blue Ink

Completed application form DS-82:
o Complete the new Online Version of the DS-82 form, (Complete the form online then click CREATE FORM button) You MUST print and submit ONE of your completed application DS-82
o A letter addressed and dated to ""Department of State"" indicating in detail the need for a second passport and must be signed by the applicant.
